Form 1583 — Application for Delivery of Mail Through Agent — is the standard USPS form CMRAs collect from every customer.
It confirms:
- Your legal identity (with two pieces of ID).
- Your authorization for the CMRA (us) to handle mail on your behalf.
- The address where the mail will physically be held.
We fill in most of it for you. You just sign and provide ID.
